Job Description

LOCATION:          Special Education – Multiple Locations

BASIC FUNCTION:

The Transition Coordinator will work with programs within Capital Region BOCES to provide technical assistance, support, consultation, and training to teachers and related service providers related to CDOS and SACC credential requirements, post-secondary transition, work-based learning opportunities, and IEP transition writing. 

RESPONSIBILITIES:

  1. Create, implement, and monitor systems for documenting transition and credential  requirements for graduation in compliance with NYSED and federal guidelines.
  2. Implement systems to track student progress towards credential requirements.
  3. Monitor work-based learning experiences in accordance with NYSED regulation.
  4. Provide consultation and organizational support to create transition and work-based learning opportunities for students.
  5. Facilitate and organize post-secondary transition plans in coordination with CSE chairpersons, teachers, and/or school counselors.
  6. Conduct and/or provide training in career assessments.
  7. Provide training and staff support for the development of transition sections in the IEP.
  8. Review transition goals and provide feedback for implementation and improvement.
  9. Assist in the coordination of post-secondary services for students and families including but not limited to ACCES-VR, OPWDD, etc. 
  10. Stay current on NYSED graduation requirements.
  11. Build and maintain partnerships with local businesses and community organizations.
  12. Other applicable duties, as assigned.
Job Qualifications
  1. New York State Professional or Permanent teacher certification in Special Education, Students with Disabilities (grades 5-9 or 7-12), Students with Disabilities (All Grades) or New York State Professional or Permanent certification as a School Counselor, School Psychologist or related Pupil Personnel Services area certification. Work-based learning programs extension is a plus but not required. 
  2. Minimum of five years of experience providing instruction and/or pupil personnel services at the secondary level.

The selected applicant will be subject to a fingerprint supported criminal history background check in accordance with SAVE Legislation effective July 1, 2001.
